However, when viewed closely, the process is often fraught with potential failure points. Challenges exist for patients of all ages, from neonates to older adults. Preterm and critically ill infants and children often require additional calories, protein, or other nutrients due to higher metabolic demands. Consequently, unfortified human milk or standard ready-to-feed infant/pediatric formulas may not provide the level of nutrients required. Therefore, hospitals often add fortifiers (e.g., calories, protein, vitamins) to prepared feedings or human milk, or they use powders, concentrates, and/or modulars (i.e., formulas with modifiable nutrient amounts) to create facility-prepared formulas. Occasionally, adult patients require specialty-prepared formulas and/or the use of modular components as well. Similar to medication compounding, at any step of the process, practitioners may inadvertently use the wrong, expired, or recalled ingredients, putting the patient at risk. Even when pre-made EN formulas are available, there is still the risk of a practitioner administering a wrong, expired, or recalled formula.</p>

The authors noted that documentation improved significantly with the implementation of bar code scanning for enteral feedings within their organization.  They noted that use of scanning could prevent formula misadministration and ensure optimal delivery as a strategy to prevent malnutrition.</p>

Among the many recommendations, nutrition and lactation support are promoted including the recommendation that the facility will utilize a dedicated space away from the bedside for feeding preparation, develop standardized feeding protocols, and have procedures in place for accurate verification of human milk AND formula to avoid misappropriation.</p>

The authors reviewed research considering that expressed human milk is typically not fed at the time it is pumped and that this “mismatched” milk may in turn potentially contribute to sleep problems and decreased physiological attunement with their mothers and environment.  Recommendations for future research, practice, and policy are provided.</p>

  <p style="font-size:17px;line-height:1.68;color:#46586f;margin:0 0 18px;">This article reviewed the aspects of the comprehensive, multi-disciplinary lactation program implemented to help improve breastmilk availability for very-low-birth-weight (VLBW) infants at hospital discharge.  The program included the implementation of centralized handling with dedicated technicians of all human milk and formula feedings as well as bar code scanning within the preparation room and at bedside.  Other program initiatives included use of a bedside lactation care algorithm, staff training, use of a lactation bundle for families, and expansion of the breast pump program.  The authors found that lactation rates at hospital discharge for the VLBW population increased significantly as a result of the interventions.</p>
